Mechanical Behavior of the Coracoclavicular Ligament and Novel Anatomic Reconstruction Complexes

چکیده
INTRODUCTION Acromioclavicular (AC) joint separations are common injuries seen in athletes or laborers in which the AC capsule and coracoclavicular (CC) ligaments are both ruptured [1]. Both surgical and conservative treatments have similar outcomes [2], with both treatments having no guidelines for rehabilitation. The CC ligaments, trapezoid and conoid, have greater structural properties than current surgical treatments [3,4], which replace the multi-bundle orientation of the CC ligaments with a single orientation [5]. Recent studies have also reported salvage of failed CC ligament repairs using tendon allografts [6]. Therefore, the objective of this study was to evaluate the mechanical behavior of the CC ligament and novel anatomic reconstruction complexes. This study can provide guidance to improve the outcome of surgical treatment and allow early mobility, accelerated rehabilitation and earlier return to sports for high level athletes or return to work for heavy laborers.

A new technique in spring ligament reconstruction using medial half of posterior tibial tendon is demonstrated as a means of supporting the arch. In addition a new concept of double bundle PTT reconstruction based on anatomical attachments of original PTT is presented with the goal of obtaining the full function of PTT.

Anterior cruciate ligament (ACL) reconstruction surgery has significantly evolved in recent years. This has led to development of new technologies that facilitate the diagnosis of ACL injury and the application of state of the art methods for treatment. In particular, individualized anatomical ACL reconstruction aims to restore native ACL function. Treatment is tailored to each patient based on...
متن کاملModified Internal Fixation Technique for Acromio-Clavicular (AC) joint dislocation: The “Hidden Knot Technique”
Acromioclavicular (AC) joint injuries are common and often seen in contact athletes, resulting from a fall on the shouldertip with adducted arm. This joint is stabilized by both static and dynamic structures including the coracoclavicular (CC)ligament. Most reconstruction techniques focus on CC ligament augmentation as the primary stabilizer of the AC joint. The best surgical technique for some ...
متن کاملComparison of Acute Acromioclavicular Joint Dislocation Treatment by Screw Fixation with and without Ligament Reconstruction
Background & Aims: Acute acromioclavicular (AC) joint dislocation is common in shoulder injuries. Considering the biomechanics of this joint, the use of a treatment method, which can restore joint physiology and biomechanics to the extent possible, is important. This study aimed to compare the treatment of acute AC joint dislocation by screw fixation with and without ligament reconstruction. Me...

The purpose of this study is to evaluate the biomechanical properties of stainless steel and bioabsorbable screw fixation of the clavicle to the base of the coracoid. Seven matched pairs of fresh frozen shoulders were prepared by removing all soft tissue except the acromioclavicular and coracoclavicular ligament complexes. The shoulders were randomly selected and fixed with 4.5-mm stainless ste...
